Thèse soutenue par Monsieur DUVERNEY-GUICHARO Michel

Titre : À propos d'un jeûne volontaire de 42 jours.

Le jeûne en lui-même n'a fait l'objet que de peu de travaux scientifiques approfondis. Notre recherche bibliographique nous a conduit à constater que les ouvrages sur ce thème concernent le plus souvent des états proches du jeûne mais qui cependant en diffèrent par leur nature (grèves de la faim, dénutritions volontaires, régimes restrictifs, etc.) Aussi faut-il admettre la persistance de questions fondamentales sur le jeûne sa physiologie, ses dangers, ses éventuels effets bénéfiques.

Nous avons, au cours de notre travail réalisé l'observation d'un sujet durant un jeûne volontaire complet avec apports hydriques libres (eau de distribution uniquement), sans supplémentassions minérale, vitaminique aux autres. La durée de l'expérience a été de 42 jours, chez un homme de 42 ans ayant déjà pratiqué des jeûnes antérieurs de longue durée, dont un de 40 jours.

Certains des paramètres que nous avons retenus avaient déjà été suivis lors de travaux antérieurs portant sur des situations proches de celle du jeûne. Nos propres résultats recoupent ceux déjà enregistrés et confirment la capacité d'adaptation du corps :

 - sur le plan du métabolisme énergétique par exemple, nous retrouvons l'augmentation des corps cétoniques et des acides gras au niveau sanguin, la stabilisation de la glycémie.

 - concernant le métabolisme protéique, on note en particulier la limitation du catabolisme mise en évidence par le dosage de la méthyl-histidine urinaire qui est basse, l'augmentation connue de l'uricémie. les modifications de certaines protéines sanguines.

 - l'équilibre hydre-électrolytique est préservé : les courbes montrant la baisse de l'excrétion urinaire des différents ions sont à cet égard très significatives.

Notre étude a un caractère plus original en ce qui concerne les vitamines et les oligo-éléments. Si l'élévation de la zincémie avait déjà été relevée, nous n'avons pas trouvé de références pour les autres oligo-éléments et les vitamines au cours du jeûne. Nous avons noté l'absence de signe carentiel clinique et biologique pour le Fer, le cuivre, le manganèse le sélénium, les vitamines A, B2, B6, Bg.

Notre observation clinique nous a permis de noter l'amaigrissement et la fatigabilité importante, la baisse de la tension artérielle et l'existence d'une hypotension orthostatique quelques paresthésies et des phénomènes de crampes au niveau des membres inférieurs.

Ces troubles n'ont pas empêché le sujet de conserver jusqu'au 42e jour une activité physique modérée soit environ une demi-heure de marche quotidienne.

Les examens paracliniques ont donné les résultats suivants :

- le métabolisme de base semble s’abaisser,

- l’électrocardiogramme ne se modifie pas,

- l’électro-encéphalogramme au 24e jour reste inchangé,

- l’étude des fonctions supérieures ne montre aucune atteinte de leur niveau malgré des performances diminuées par la fatigabilité après la 3e semaine,

- l’examen des fonctions sensorielles, vision et audition est sans anomalie, l’olfaction étant de façon subjective très développée,

- sur le plan de l’immunité, l’observation des populations lymphocytaires ne révèle aucun changement significatif, les tests cutanés d’hypersensibilité mettent en évidence des réactions très faibles après trois semaines.

Aussi, à la question essentielle de savoir si des troubles graves cliniques, paracliniques ou biologiques pouvant mettre en jeu la vie du sujet ont été démontrés, nous serions tentés de répondre négativement.

Ceci évidemment ne permet pas de conclure péremptoirement à leur inexistence ni de réfuter la possibilité, à tout moment, de complication brutales et très graves. Nous éviterons par ailleurs de généraliser des résultats obtenus à partir d’un cas isolé, de même que nous ne porterons pas de jugement sur l’innocuité ultérieure de cette expérience. Cependant, cette étude, en particulier par l’observation de certains paramètres originaux comme les oligo-éléments et les vitamines peut, nous l’espérons, contribuer à une meilleure connaissance des phénomènes d’adaptation du corps et des complications éventuelles liées à l’état du jeûne.

Nous avons enfin au long de cette étude évoqué à quelques reprises la possibilité d'un intérêt thérapeutique à la pratique du jeûne. Sans revenir ici sur des points particuliers pouvant présenter dans ce sens quelques aspects positifs, nous voulons envisager de façon plus générale cette question.

Ainsi pourquoi ne pas supposer que les modifications métaboliques induites par le jeûne puissent comporter quelques bénéfices ?

Ne peut-on imaginer que cette forme «d'autolyse sélective» que constitue le jeûne permette dans un temps limité un «apport nutritionnel» plus adapté aux besoins des cellules impliquées dans des fonctions essentielles de l'organisme? L'évolution de l'état du zinc n'est-elle pas à cet égard démonstrative?

Telles sont certaines des voies qui, à notre avis mériteraient d'être explorées plus avant. En définissant plus précisément le champ de travaux de recherche, la fonction immunitaire par exemple, en limitant la durée du jeûne à quelques jours, en élargissant au contraire la population étudiée, il serait sans doute possible de déboucher sur des conclusions intéressantes.

On peut noter que le docteur Duverney-Guichard reste prudent puisqu'il s'adresse à des médecins, sachant que le corps médical n'est pas très ouvert à de nouvelles approches "alternatives". Néanmoins, les résultats positifs de ses observations sont très évidents et convaincants.
